Tax Appointment Document Checklist

Organize your records before the appointment, but do not send tax documents or identity numbers through the public website.

Confirm the appointment scope first

Tax preparation is separate from notary service. Before sharing sensitive records, confirm that the preparer accepts your return type, filing year, state filings, business or self-employment activity, investment activity, credits, deadlines, and any prior-return issue involved.

Also confirm preparer credentials, fees, secure document-delivery method, expected timeline, e-file process, and how questions after filing are handled.

Identity and household information

Gather government-issued identification and taxpayer identification information for each person included on the return. Organize legal names, dates of birth, filing status information, dependent residency details, and childcare-provider information when relevant.

Do not enter Social Security numbers, identity-document numbers, or dependent information into the website request form, ordinary email, or text. Use the secure method confirmed by the preparer.

Income records

Collect every income statement received for the filing year, including corrected forms. Common records include W-2 wages, 1099 contractor or platform income, interest, dividends, investment sales, retirement distributions, unemployment, Social Security benefits, rental records, and other taxable income.

Compare the forms against your own records and note any missing or incorrect statement so it can be resolved before filing.

Potential deductions and credits

Organize education costs, student-loan interest, retirement contributions, charitable gifts, health coverage information, medical expenses when relevant, mortgage and property records, energy improvements, dependent-care costs, and business expenses supported by records.

A checklist does not determine eligibility. The preparer should apply current federal and state rules to the verified facts and explain any additional documentation needed.

Self-employment and small-business records

Bring income totals, payment-platform forms, invoices, bank records used for bookkeeping, mileage logs, equipment and asset purchases, home-office details when relevant, estimated-tax payments, and categorized expenses supported by receipts or records.

Avoid handing over an unorganized box of mixed personal and business activity at the deadline. A clean summary plus underlying evidence improves accuracy and makes follow-up easier.

Prior returns and filing logistics

Bring the prior-year federal and state returns, notices from tax agencies, estimated-tax records, and direct-deposit information through the approved secure channel. State any address, marital-status, dependent, employment, or business change.

Review the completed return before authorizing filing. Keep a final copy, supporting records, confirmation of acceptance, and the preparer’s contact information according to applicable retention guidance.

Protect tax information

Tax records contain identity and financial data that can cause serious harm if mishandled. Verify who will receive the documents, how they are transmitted and stored, and how long they are retained. Do not use the notary appointment form as a tax portal.

If secure delivery is not yet configured, call the business for instructions rather than improvising through consumer messaging or public links.

Before transmitting records, confirm the preparer, secure delivery method, scope, fee structure, and deadline. A request for Social Security numbers, tax forms, or banking details through the ordinary notary website form should be treated as the wrong channel.

Use the appointment to disclose the full filing situation rather than only handing over forms. Life changes, notices, business activity, investments, multiple states, and missing records can affect the return and the type of preparer needed.
